“Volunteer Worker Agreement”

As a volunteer worker on the premises of the San Antonio Food Bank and assisting the Food Bank sort its food products or perform other volunteer services, I understand that I am not eligible for benefits under workers compensation insurance in the event of bodily injury to myself.

I understand that as a SAN ANTONIO FOOD BANK volunteer I will not be paid or compensated for my services.

I have entered the premises of the San Antonio Food Bank of my own free will. I will assume responsibility and will hold harmless the San Antonio Food Bank for any injury that I might sustain while on said premises.

It is also my understanding that the San Antonio Food Bank agrees to hold me harmless for any accidental bodily injury or property damage done by me while on said premises.

The San Antonio Food Bank has my permission to use my picture working for the Food Bank for any Food Bank promotions.

I understand that the above information is voluntarily supplied and may be used and disclosed for Food Bank purposes.
